    I understand that the Student is registered to attend the Eastern U.S. Music Camp (EUSMC) upon receipt of the completed 2025 Camper Application and $50.00 Registration Fee. The balance is required to be paid by June 1, 2025. Applications are accepted on a first-come, first-served basis.

    Camp/University Regulations/Certification Policy:

    I understand no refunds or deductions for the cost of the program will be made for any reason including cancellation, late arrival, early departure, sickness or expulsion for disciplinary reasons. All payments will be refunded if the program is cancelled due to COVID-19. All EUSMC participants are required to comply with Camp and University rules and regulations and any federal, state, and local law. Rules will be distributed and explained upon arrival in Hamilton. Violations or misconduct will result in immediate dismissal from Camp without refund. The program is inspected and certified by the New York State Department of Health for children's camps. EUSMC does not discriminate on the basis of sex, race, color, national or ethnic origin, religion or physical handicap. The information contained in the EUSMC website is subject to change without notice.

    Camp/University COVID-19 Regulation Policy:

    I understand all EUSMC participants must abide by all Camp and University policies and comply with directions of Camp and University officials acting in performance of their duties. This could include mask requirements in certain spaces or during certain activities and/or testing requirements. All participants are required to provide documentation when checking in on Registration Day verifying that they are fully vaccinated, unless granted a medical or religious exemption. In the absence of specific University or Camp policy, we expect participants to follow guidance from the Centers for Disease Control and Prevention (CDC) and New York State regulations related to the health of our community for managing the presence of SARS-CoV-2 on campus and within the Village and Town of Hamilton. The music program is subject to change to meet Centers for Disease Control and Prevention (CDC), New York State Department of Health and/or Colgate University COVID-19 guidelines.

    I understand that this application is being submitted electronically and that my typed name below shall constitute my signature on this application. My signature and submission of this Student Application indicate that I agree to the "Terms and Conditions for Enrollment". I authorize the use of photographs/videos taken during the EUSMC season and give permission and consent that any such photographs/videos may be published and used to illustrate the EUSMC.
